Our therapist will provide a detailed letter summarizing your therapeutic journey. This includes a brief overview of your symptoms, the dates of your sessions, and any specific reasons for referral, if applicable.
The letter serves as a comprehensive record of your progress and can be shared with other professionals or organizations as needed, ensuring continuity of care and support.
RM30 per letter
Our therapist will provide a concise report that highlights essential details, including the issues discussed, relevant background information, a summary of assessments performed, a final diagnosis (if applicable), and brief recommendations for future steps.
This report serves as a clear and professional summary of your therapeutic journey, ensuring you have the necessary information to move forward with confidence.
RM90 per report
This service is available exclusively for clients who have completed a battery of psychological assessment.
The comprehensive report includes a detailed breakdown of the presenting issues, relevant background information, findings from the mental status examination, descriptions and results of each assessment, a final diagnosis (if applicable), and personalized recommendations.
Designed to provide clarity and direction, this report serves as a valuable tool to support your next steps and ensure continuity of care.
Inclusive with assessment package
Kindy note that the preparation of letters and reports are guided by the therapist’s professional judgment and discretion. In certain cases, our therapists may decide not to issue a report if they believe it could be inappropriate, unethical, or not in the client’s best interest.
